Nutrition for Dry Eye: The Omega-7 Story

You have a bottle of drops in every bag you own, they work for about twenty minutes, and by four in the afternoon your eyes feel like they have sand in them again. Sea buckthorn, spelled seabuckthorn on plenty of labels, comes up constantly in that search, usually next to the word omega-7 with no explanation of what that is.

So here is the explanation, starting with the tear film rather than the berry.

Your tear film is not water. It is a layered film with an oil layer on the outside, and your body builds that oil layer out of fatty acids that came from food.

What is a tear film actually made of?

Three layers, working as one surface.

Against the eye itself there is a mucin layer, secreted by goblet cells in the conjunctiva, and it is what lets watery tears spread evenly instead of beading like water on a waxed car. Above that sits the aqueous layer, the part everyone thinks of as tears. On the outside, in contact with the air, sits a thin lipid layer from the meibomian glands along your eyelid margins. That oil is the lid. It slows evaporation between blinks.

When the film evaporates faster than it is replaced, what is left behind gets saltier. That rise in tear film osmolarity is one of the things clinicians actually measure, and it is behind the burn, the redness and the gritty feeling you get in winter, in air conditioning, and after eleven hours at a screen.

Your body rebuilds all three layers continuously. Two of the three are built from material you eat.

What is omega-7 and why is it rare?

Omega-7 is palmitoleic acid. It is a monounsaturated fatty acid, present throughout your own tissue, and genuinely uncommon in food. Most of the fat in a normal diet is omega-6, with some omega-3 and omega-9, and almost no omega-7.

Fatty acids matter here for a structural reason rather than a mystical one. Cell membranes and secreted lipids are built out of fatty acids, and the profile your body builds with is shaped by the profile you supply. Mucous membranes, the moist epithelial linings of the eye, mouth and gut, are exactly where that material question shows up.

Sea buckthorn is one of the few foods where omega-7 sits in the fruit pulp in real quantity, alongside omega 3, 6 and 9 in the same berry.

What has the sea buckthorn research on dry eye actually measured?

One trial is worth naming precisely, because it is the one everything else in this category quietly refers to.

Petra Larmo and colleagues at the University of Turku ran a double-blind, randomised, placebo-controlled trial published in The Journal of Nutrition in 2010. One hundred adults with dry eye symptoms took either 2 g of sea buckthorn oil or a placebo oil daily for three months, running from autumn into winter, and 86 completed it. Tear film osmolarity rose in everyone across the cold season, which is what you would expect. It rose significantly less in the sea buckthorn group. Redness and burning intensity trended lower.

Now the honest part, because it matters more than the finding. That trial used 2 g of a concentrated sea buckthorn oil, a fraction pressed out of the berry, and not our purée. One trial in 86 people is a signal, not a settled fact, and we are not going to dress it up as one. What it establishes is that the berry's fatty acid profile is a serious enough nutritional subject that a university ran a controlled trial on it.

You can have the molecule alone, or the molecule in the food that made it. Only one of those is something humans have eaten for as long as there have been humans.
